Pediatric Oral Surgery

Supernumary Teeth / Mesiodens

A mesiodens, also known as a supernumary tooth, is an extra tooth located in the middle of the upper jaw behind the incisors. It is most often found on routine x-rays and is usually asymptomatic. Sometimes the extra tooth will erupt like a normal tooth, but it should be monitored closely. If the tooth does not erupt and is left untreated, this extra tooth can lead to crowding of the teeth or even prevent the eruption of the permanent teeth. Typically, the tooth is removed between the ages of 6 and 8 if asymptomatic. Removal of the tooth is usually done while the patient is asleep through a small incision on the top of the mouth. Stitches are typically used to hold the gums in place during the healing process. Once the tooth is removed, the permanent teeth will erupt in their normal pattern.

Ankylosed Primary Teeth

Impacted teeth are usually found in the permanent dentition. Very rarely is a primary tooth impacted. The most common primary tooth to be impacted is the primary second molar. Primary teeth become impacted for various reasons which include: trauma to the tooth bud or by a mechanical obstruction. In either case, it is very important to closely follow the eruption of the primary tooth. If the primary tooth remains impacted, it may cause a delay in the eruption of the permanent tooth. Typically, if the primary tooth is impacted, the tooth is removed while the patient is asleep, and a space maintainer is used to prevent movement of the adjacent teeth and allow room for the permanent tooth to erupt.


When it comes to kids, our priority is to make a safe and comfortable environment for them during there visit with us. We employ a combination of liquid sedatives, nitrous oxide (laughing gas) and IV medications to ensure the child is unaware during the procedure and has a good experience. The surgeons and surgical assistants are all life-support certified and vital signs are monitored throughout the sedation.

My son had an impacted canine in the bone that needed to be extracted. I was recommended by a friend who had lots of dental work done. Along with the other great reviews, and having someone I know having personally recommending this place, I had a consultation with Dr. Zachary. I had questions and concerns. This's my baby after all and I was nervous. Dr. Zachary was understanding but not a (ya ya ya) way. He was confident but not cocky. I finally found a place where I felt as comfortable as a mom could feel, knowing her son was about to be put under for oral surgery. The staff was compassionate and accommodating. My son is now recovering and has little pain. I got a call the next morning by his recovery nurse Annie, which btw is a sweet heart. Overall great experience. My daughter was well taken care. My daughter was really scared and nervous. Dr. Zachary spoke with us and did not rush me out of the room to get started. Dr. Zachary did not rush the procedure. If we need any oral surgery again we will go back.

I wanted to take a moment to thank Dr. Choi and his staff for taking such wonderful care of my daughter who needed a surgical dental procedure in December 2016. My daughter is 11 years old and has autism. She needed some of her teeth extracted so that we could proceed with her orthodontic treatment. It was very important to me to select a Dr. who had experience working with special needs children and was familiar with some of the challenges a procedure such as this can pose for them. Dr. Choi was very caring and explained things to her in a very calm and caring manner. I was extremely pleased with the entire process from the initial consultation to the surgical procedure itself. The office is lovely and the staff were very helpful and friendly! The surgery was fast and my daughter recovered very quickly with minimal pain and discomfort. In fact, the day after surgery, her orthodontist put on her aligners. Without this procedure we couldn't have moved forward successfully in her treatment. She is now on her way to a beautiful smile.
